How To Register Your Child for Summer Camp

Registration and payment can be completed online with a Debit Card, Credit Card or Pay Pal. All payments must be paid and proof documents emailed to Rjackson@opajax.org before the first day of camp, no exceptions. We have scheduled an on-site registrations (if seats are still available) from 9 a.m. to 10:00 a.m.  You can drop-off proof documents as well.

Ensure you have the following before you start:

01.

Your child’s public school student ID#. (If your child is home schooled or is entering Kindergarten in the fall please call our office for instructions BEFORE you start this application.)

02.

The last four digits of your child’s social security number.

03.

Your child’s Insurance Policy/Medicare number.

04.

Have your Debit Card, Credit Card or PayPal account on hand. 
If you are paying with Cash app, place child’s name in the “for” field so your payment will be correctly applied.

05.

The registration MUST be filled out by the child’s Parent/Legal Guardian!

06.

E-mail your proof documents to Rjackson@opajax.org, or drop them off at our on-site registration day.

If you prefer to register in person, please print a manual application from the “Summer Camp” page and bring it along with supporting documentation and payment to the scheduled onsite registration.
